Back To School Forms

Frequently Asked Questions

Q: Why are some forms not pre-populated?
A: Some optional forms are currently not automated yet within the system. Continuous improvements occur annually which will eventually incorporate all form fields.

Q: What is the benefit of using this system?
A: There are typically over 500 fields that parents manually fill in yearly. Many of these are redundant. We have consolidated the information to allow faster entry for our parents.

Q: How can I correct typo's after the forms are printed?
A: Log back into the site, click on the green subject title link(s) next to the check marks and make your alterations. Reprint the forms when you are confident that all the information is correct.

Q: Why do we need to print the forms if we are filling them out electronically?
A: It is a state requirement that hard copies of the forms be present on school property.

Q: When I printed the forms, only one parent's name is listed to sign. Is only one parent required and if not, do I physically need to sign the forms?
A: Only one electronic signature is required for form authorization. As long as (1) signature is printed on the forms, you do not need to manually sign them.

Q: I did not see a way to send these forms to you electronically?
A: All data that is entered is automatically submitted and stored electronically. Please print the forms and send them with your child on the first day of school to meet state requirements.

Q: I forgot or cannot locate my child's Student ID?
A: Click the Can't Login link directly below the logon boxes to recover the Student ID.

Q: What web browsers are supported?
A: All latest versions of Internet Explorer, Firefox, Google Chrome and Safari.

Q: How long will this take to fill out?
A: It will take between 2-15 minutes all dependent on how much information is pre-populated for you.

Q: What if I can't complete the forms in one sitting?
A: Simply click the LOGOUT button and return whenever it is more convenient for you.

Q: What do I do with the forms once printed?
A: Give them to your child to bring to the first day of school.
